Lost passports and missed flights get the attention. The mistakes that actually damage most trips are smaller and more repetitive, and nearly all of them are avoidable with a single decision made earlier.
Booking the cheapest flight without checking the arrival time
A flight that lands at 01:30 saves ninety euros and costs you a night's sleep, an expensive taxi and the first morning. Calculate the true cost before congratulating yourself on the fare.
Booking accommodation by price per night alone
A place twenty minutes further out at fifteen euros less per night will cost you forty minutes a day in transit plus the transport fare. Over a week that is nearly five hours. Look at the location on a map relative to what you plan to do, every time.

Over-scheduling the first day
You will be tired, possibly jet lagged, and unfamiliar with the transport. Plan almost nothing. A walk and a meal is a complete first day and it sets the trip up properly.
Not checking closing days
Museums close on Mondays across much of Europe. Many close on Tuesdays in France and Italy. Small restaurants close one day a week and often on Sunday evenings. Two minutes of checking prevents the classic wasted morning.
Paying in your home currency abroad
When a card machine or ATM offers to charge you in your home currency, decline. The exchange rate applied is set by the terminal operator and it is worse than your bank's, usually by three to six percent.

Assuming the famous thing is the best thing
Sometimes it is. The Alhambra deserves its reputation. But in most cities the second most famous museum has the same collections with a tenth of the crowd, and the third best viewpoint is the one locals use.
Treating a guidebook restaurant list as current
Restaurants change hands, chefs move, and a place that was excellent three years ago may now be trading on the recommendation. Check recent reviews with dates, not overall scores.
None of these are catastrophic on their own. Three or four together, though, and a trip starts to feel like an errand. The fix in every case is a small amount of checking before you commit money.
